It is a medical emergency. Viral cases can be milder; bacterial meningitis needs immediate hospital care. We do not wait at home to ‘see if the fever breaks’ when the neck is stiff.

A child who is drowsy, irritable on being held, or vomiting without diarrhoea.
Pain on bending the neck, photophobia and a full fontanelle in babies.
Viruses, bacteria and, less often, other organisms. Vaccines prevent several of them.
Hib, pneumococcal and meningococcal vaccines save lives.
Alertness, rash, neck and fontanelle are checked immediately.
Suspected bacterial meningitis is not managed with a clinic antibiotic alone.
Household members may need advice after a confirmed case.
Hearing and development are reviewed once the child is home.
We take a full history — onset, feeding, fever, sleep, school and any earlier reports.
Growth, chest, tummy, skin and development are checked. Tests are advised only if they change care.
A clear home plan is given, with a review so we know the child is settling.
Go to emergency care now for fever with a stiff neck, purple rash, or a baby who is floppy and not feeding.
